It is just the second time I left a memorial death katana and continue to try beat the level. I accidentally restart and/or get out ot the area by mistake and when I'm back there, the sword is there when I died, but is not a red memorial anymore. I think, it could be linked to the fact I didn't reach/saved my progress in the shrine or something related to the new "if the area is edited former katanas dissappear" update. :/

And now, after exiting and re entering BK (T-T) that second katana is back there... Is my game load haunted? Who I gonna call??...

Hmm.
We will look into that - sounds like there might be an issue there somewhere.

The expected behavior is that you may only have a single 'memorial Katana' in an adventure - so if you add another, only your latest addition will be saved.
However, as long as you are staying in the same adventure, you will see your older swords - but a restart should show you only your latest one.

the second time I mentioned was in a totally different adventure. But while later both me and mostro found the first katana active there. I think something similar happend to him in some other area
